Q:  Who is eligible for ADA(Americans with Disabilities Act) transportation?

A:  Any resident of Polk County may request an Eligibility Application by calling (863)534-5500 or clicking on ADA Application Form.

Q:  How do I or how long do I have to cancel my appointment?

A:  Trip cancellations must be made at least 24 hours in advance of the scheduled pick-up time. If you fail to cancel the trip 24 hours in advance or if you are not there when the vehicle arrives, your trip is counted as a “No Show.”

The No-Show policy states that three (3) no-shows in a 3-month period can result in suspension of your services.

Please let us know as soon as possible if you will not be able to keep your scheduled appointment.

Q:  Who is eligible for elderly services transportation?

A:  Persons over the age of 60 who participate in various programs, such as Fellowship Dining or Adult Day Care, may be eligible for transportation. Your case manager from Polk County Elderly Services will arrange for transportation if it is part of your care plan. You may request transportation through the Elder Helpline by calling (863)534-5320.

Q:  Who is eligible for Medicaid non-emergency medical transportation?

A:  Individuals who have a Medicaid card and need transportation for a Medicaid eligible service may qualify for Medicaid transportation. If you have not been approved for Medicaid, you will not be eligible for Medicaid transportation. If you are unable to provide for your own transportation, call (863)534-5500. An assessment can be scheduled to determine if you are eligible.
