The Health Insurance Portability and Accountability Act of 1996 (HIPAA) was passed to, among other things, establish national standards for privacy and security. To meet the requirements of HIPAA, the Board of County Commissioners has made changes in its practices and has provided extensive training to staff in how to carry out any HIPAA-related activities.

This website has our HIPAA Policies and Procedures, along with all the necessary forms. In each section, there is a description of the Policy, the related Procedure, and reference to the regulatory authority. At the end of each section are Frequently Asked Questions that give further information about the subject.

You may use the site in the following ways:

  1. Go to the appropriate chapter and click on the subject. You will find extensive information on what to do. For each procedure that requires a form, click on the link within the chapter to go to the appropriate form.
  2. If you know the form you need, you may by-pass the policy and procedures by clicking on the form name in the table. This will link you directly to the form itself.
