Elderly Services provides Case Management to eligible individuals, frail, elderly, aged 60+, and disabled adults (18-59) through initial and ongoing evaluation of a client's emotional, physical, medical, environmental, social, and mental condition. These services are provided in conjunction with support system(s) the client may be receiving. The case manager assists clients in arranging for services which allow clients to remain in the least restrictive environment and to preserve their dignity. All services are processed through the West Central Florida Area Agency on Aging.
Other services provided include Case Management, Adult Day Care, Chore Services, Emergency Alert Response Services, Homemaking Services, Personal Care Services, Nutrition Services, Respite Care, Transportation Services, Emergency Home Energy Assistance for the Elderly, the Elder Help Line, and Information and Referral.
