All employees should be encourages to report any injury immediately to their supervisor. Failure to file a First Report of Injury within seven (7) days can result in a $1,000.00 fine for each
late filing.
Steps to Filing Workers Compensation
- Complete a First Report of Injury to Risk Management or our workers' compensation carrier within seven (7) days of first knowledge of injury.
- Employee must sign the First Report of Injury form. *
- Immediate supervisor should call CorVel at 1-800-704-2433 - a nurse will direct the medical care to the preferred provider.
* If employee is unavailable to sign his/her name on the First Report of Injury; type in "employee unavailable for signature" and send form to Risk Management.
Worker's Compensation Pharmacy Program
Beginning January 1, 2010, Polk County Board of County Commissioners will be administering their workers’ compensation prescriptions through myMatrixx, instead of CorVel.
If an employee has an on the job injury, that requires medical care, please print this prescription information, so the employee may take it with them to the pharmacy, when filling their initial workers’ compensation prescriptions. The injured employee will receive a permanent prescription card, specific to their injury, within 3 to 5 business days.
