Please try to provide the following information when contacting the I.T. Service Desk.

1. Provide your name or the name of person the request is for.
2. Provide a telephone number where you can be reached.
3. Provide your e-mail address.
4. Provide your general location (what building you're located in?).
5. Please describe in detail the problem/request/reason for calling.
6. Provide what type of hardware you are using (if applicable).
