Polk HealthCare Plan
The Polk County HealthCare Plan pays for health care services for Polk’s low and limited income residents who do not have insurance or who have inadequate insurance coverage. Medical services provided by the Polk HealthCare Plan are coordinated through a participating primary care doctor and may include:
- Doctor visits and checkups
- Specialty care
- In-patient services
- Urgent care
- Emergency room care
- Prescription medications
- Medical tests

Polk County Health and Human Services and Indigent Health Care want to help you connect with the services you may need both during and after the COVID-19 event. Calling 2-1-1 will connect you with services available from county and city governments as well as local nonprofit organizations.
