Outreach and Education
The mobile unit is available at various locations throughout the county. You can find the schedule on this website, under events.
For those residents who are interested in healthcare resources provided by the county, staff will conduct eligibility interviews for the Indigent Health Care (IHC) programs. If you do not qualify for the Polk HealthCare Plan, mobile unit staff has referral information/materials on hand to direct you to any number of clinics in partnership with IHC.
Diabetes Nutrition Education
UF/IFAS Extension Polk County offers healthy lifestyle and diabetes nutrition education classes to Polk County residents.

Burial Cremation Program
The Burial Cremation Program insures that every eligible resident of Polk County, without adequate resources, will receive a simple but dignified burial or cremation. Florida Statute 406.52 requires counties to bury or cremate those individuals who are low income or indigent as well as those whose families do not claim. This program provides services as a last resort. Polk County coordinates with local funeral homes and cremation services for the burial cremation program.
Emergency Food & Shelter Program
Polk County supports and manages FEMA’s Emergency Food and Shelter program, which assists qualifying county residents who have a recent loss of income or experienced an unexpected expense, with short-term financial assistance for shelter and utilities, in an effort to meet the their basic needs.
This program is able to pay for up to one month of shelter and/or utilities for residents who qualify. A resident may not utilize these services more than once a year.
